Abstract

The editors would like to acknowledge the late Kenneth Simpson for giving us the benefit of his wisdom and experience early in the preparation of this volume. Ken passed away in 2013 before the volume was completed and it is to him that we dedicate this book. We would also like to acknowledge the input of the late Professor Susan Manning who also passed away in 2013 sadly before her contribution to the volume could be submitted.
